GTA+ availability and membership requirements are governed by Rockstar's service terms. This bonus should not be confused with the Ultimate Edition Upgrade; they are separate items.

Prices and store availability can vary by platform and region, so check the relevant PlayStation or Microsoft storefront before buying. For a physical code, Rockstar recommends using a retailer in the same country as the platform account that will redeem it.
The important distinction is straightforward: Standard contains the game, while Ultimate contains the game and the Ultimate Edition Upgrade. Standard owners are not permanently locked out of the Ultimate content because Rockstar offers the upgrade separately.
